Title: Case Manager- Santa Monica HMST

Reports to: Program Manager, HMST

Status: Full-time, Non- Exempt (Hourly)

Location: Santa Monica Old Firehouse

Schedule: Monday- Friday 8:00am-4:30pm

Summary

The People Concern’s Santa Monica Homeless Multidisciplinary Street Team (HMST) is seeking a Case Manager to perform a variety of tasks to assist the most vulnerable and chronically homeless individuals in their recovery from homelessness and in regaining control over their lives. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of HMST is to target the highest utilizers of the City of Santa Monica’s emergency resources (police, fire, and hospitals). The skilled interdisciplinary team uses best practices for engaging chronically homeless individuals with co-occurring mental health, substance use, and/or medical issues. A low client-to-staff ratios is used to maximize effectiveness. The team works closely with each client to develop individualized service plans. The Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.

Essential Duties and Responsibilities

1. Utilize harm reduction, social, environmental, and motivational, strategies to support clients, in collaboration with clinicians, other counselors, case managers, and support staff

2. Encourage and promote an environment that is strength based to assist clients in meeting their individual goals

3. Street outreach to identified homeless persons working with the team to engage in services and accept treatment from the program staff assist with enrollment process as needed

4. Assist clients to acquire permanent housing utilizing a variety of resources.

5. Provide crisis intervention, basic services, and short-term and long-term case management services to currently homeless and previously homeless housed individuals.

6. Develop effective, trusting relationships with clients with a focus on facilitating their move towards independence.

7. Complete all charting in a timely manner.

8. Provide information, referrals and social services advocacy.

9. Network with agency and outside service providers, ensuring client access to appropriate resources.

10. Provide life skills counseling as appropriate.

11. Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits by targeted population.

12. Work effectively on an interdisciplinary team of a Licensed Program Manager, Physician Asst, Substance Use Case Manager, Clinical Case Manager, Psychiatrist, and Peer Advocate.

13. Other duties as assigned.

Qualifications

1. B.A. or equivalent experience.

2. Minimum of two (2) years in case management.

3. Demonstrated knowledge of issues faced by population served (homelessness, serious mental illness, substance addiction, and physical health problems).

4. Skilled in non-violent crisis intervention.

5. Ability to self-motivate, work independently, and communicate effectively.

6. Able to work collaboratively as part of an interdisciplinary team

7. Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record

8. Computer literate; able to use computerized database system for client file management.)

9. Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers

10. Computer literacy, including knowledge of Microsoft Word, Outlook and Excel

11. Detail oriented with excellent time management, organizational, written, verbal, and computer skills
