Role Summary 

The Registered Dietitian will be at the forefront of this intensive, adaptive, and patient-focused model. Panoramic Health is looking for nutrition professionals that understand the importance of the “Quadruple Aim”, where high-touch interactions between patient and healthcare provider enhance patient experience, improve health outcomes, reduce costs, and improve the work life of the provider. 

As an RD, you will manage the nutritional needs of a caseload of patients as they navigate the staged progression of chronic kidney disease (CKD). This will be done by providing disease-specific education in class-based settings, intensive support for nutritional issues and the management of non-medical risk factors as barriers to positive health outcomes. RD’s will further engage patients considered high risk for advancing CKD progression and unplanned dialysis starts, in one-on-one or telehealth encounters, to provide focused nutritional status assessment customizing nutrition recommendations to develop and implement nutrition care plans in collaboration with the interdisciplinary team based on patient risk level CKD/ESRD patients.  

Responsibilities 

This type of intensive service provision will require dietitian professionals that: 

  • Can build strong, productive relationships and maintain consistent communication with patients, their families, and providers. 

  • Manage a caseload of nephrology practice patients and perform monthly engagements and on- on-one/ in-person nutritional coaching sessions with actionable interventions based on patient risk. Advise patients and their families on healthy lifestyle and nutrition principles, individualized nutrition plans and modifications including food selection and preparation.  

  • Direct patients to and distribute approved educational materials concerning nutrition and positive lifestyle change for optimizing patient knowledge and actions to positive health outcomes.  

  • Address barriers and access to healthy food and nutrition supplements, weight management needs, nutritional management of comorbid conditions, negative behaviors patterns and any other risk factors that affect health outcomes in conjunction with other members of the care coordination team. 

  • Routine remote engagements with patients starting dialysis, both planned and unplanned. Engagements will be designed to ensure a smooth transition to dialysis and nutrition changes in collaboration with the in-center RD.  

  • Serve as the nutrition expert to the assigned Value Based Care team(s), keeping the team educated on current nutrition and industry trends via professional organizations related to nutrition and kidney disease (i.e., Academy of Nutrition and Dietetics, National Kidney Foundation, American Kidney Foundation) 

 

Education, Certifications/Licensure: 

  • Bachelors (Required) 

  • Active Registration by the Commission on Dietetic Registration (CDR) (required) 

  • Licensed in Nutrition/Dietetics as required by the applicable state 

 

Work Location:  

  • 90% Work from Home 

  • 10% On location (Nephrology Practice Office or other site for group education classes)
