Registered Nurse Telephonic Triage: Boca Raton, Broward County, Deerfield Beach, Delray Beach, FL

The RN Triage for will maintain the primary responsibility of assisting the Circle of Care (CoC) team and patients by performing telephonic patient assessments for inbound and outbound calls and by serving as a resource to other CoC team members.  Home and Virtual visits will be required to back up the RN Case Managers in the event sick calls or other unplanned events. 

Duties and Responsibilities 

• Collaborate with Care Team and physicians to provide telephonic disease management to prevent readmissions, future hospitalizations, and improve patient care. 

• Hold phone and video patient consultations for chronic and episodic events and determine the appropriate level of care required to safely meet the patient’s medical needs. 

• Enhance a collaborative relationship to maximize the patient’s/family’s ability to make informed decisions about next steps in their care. 

• Schedule medical appointments and direct patients to doctors, community services, experts, or other resources. 

• Act as a patient’s advocate, liaison, and information resource; escalating all appropriate information to their nephrologist as appropriate. 

• Assess and assist patients for linkage and utilization of appropriate medical and community resources in conjunction with other members of the care coordination team. 

• Complete timely and accurate documentation per company-documented policies and relevant professional and regulatory standards. 

• May be required to backfill home and virtual visits on occasion to backup the RN Case Managers on an as-needed basis. 

• Perform other duties and responsibilities as required, assigned, or requested. 

• Facilitate projects and presentations designed to develop and improve the role of the Triage RN role in the value- based care programs 

• Act as resource and representative for value-based care at health fairs and professional seminars as assigned 

• May be asked to teach virtual KDE classes 

Education/Training and Certification, Licensure, Registration Requirements 

• Degree in Nursing from an accredited institution, required.  

• Active, unencumbered clinical level of licensure in the state of practice. 

• Must be eligible or licensed in all states that patient triage. 

• Active and unsanctioned driver’s license. 

• Active BLS 

Experience 

• Minimum of 2 years of pertinent RN experience  

• Experience working with chronically ill (CKD/ESRD) patients 

• Home care, case management, and/or hospital experience preferred 

• Acute care experience within the past 5 years is preferred – ICU, CCU, ED, Stepdown, Nephrology, Renal Transplant or Dialysis RN’s
