St. Ann's Community Offers:

- Competitive Salary $16 - $20.15/hr.
· $4/hr. Evening/Night Differential
· $6/hr. All Weekend Shifts Differential

Job Description:

· Provide high quality, individualizes patient care in collaboration with our nursing team

· Take initiative while assisting and maintaining patient care

· Assist the resident in activities necessary as per individualized care planning

· Transport Elders to and from activities

· Perform evaluation and reporting

· Provide resident and family with emotional support individualized care planning

· Receive/give reports to team members/team leaders to appropriate professional staff
 

Candidate Requirements:

· Valid NYS Certified Nursing Assistant License (CNA) - Required

· Minimum of 1 year experience working with Elders - Preferred

· Ability to work in a team oriented work group

· Proficient in various computer programs and Medical Records Documentation system
