A New Leaf’s West Valley Community Behavioral Health Service (WV CBHS) in Glendale provides confidential services for children and their families. Counseling, case management, medication management and support services are provided. A New Leaf is seeking a full time Case Manager for their West Valley Community Behavioral Health Clinic.

Hourly Pay Rate: $20.00 (+ DOE)
Location: Glendale, AZ

Hybrid Schedule:

  • Monday – Friday; flexible start and end times
  • Minimum of 3 days required in-office; subject to change based on clinical needs
  • Required to work at least 1 day until 7pm

QUALIFICATIONS
Education:  
 

  • GED/HS diploma + minimum of 3 years health / human services experience OR
  • Associate Degree in related field OR
  • Associate Degree nonrelated field + minimum 1 year of health / human services experience + completion of related bachelor's degree program coursework or additional field related certifications OR
  • Bachelors degree related field OR
  • Bachelors in non-related + minimum 1 year health and human services experience
Experience:
  • Relevant experience providing family support / living skills face to face with clients, preferred

Primary Responsibilities:

  • Coordinating Child and family Teams (CFT) and serving as a facilitator and/or team member, using CFT practice guidelines and ensuring that the Plan is carried out. 
  • Developing and maintaining collaborative relationships with clients, their family members and significant support systems. 
  • Meeting frequently with clients and family members, providing face to face, telehealth and telephonic services. 
  • Provide Family Support Services and or Living Skills AHCCCS covered support services to identified clients.
  • Screening, assessing, develop, and implement treatment/service plans under the direction of the Team. 
  • Updating and completing staffing notes for multi-disciplinary clinical team reviews. 
  • Meet and sustain monthly productivity expectations as determined by clinical leadership. 
  • Manage assigned caseload to ensure compliance and coordination of care expectations are met. 
  • Collaborating with Child and Family Teams, arranging for appropriate evaluations, provide support to clients/families to ensure appropriate follow-up medication appointment, and referrals and community-based services. 
  • Coordinating comprehensive support though in-home and community-based education and interventions to address the families' goals. 
  • Reliable transportation is required as this position may drive personal vehicle to attend client visits and conduct program business, may require in home services and driving at times but not regularly.

Compliance:

  • Must be able to pass various background checks
  • Clean 39-month driving record
  • Must pass pre-employment drug test
  • Valid driver’s license with reliable transportation

  • Must Be at least 21 years of age
