Adult Health Homes Care Manager – Full Time - Flexible
JOB TITLE: Adult Health Homes Care Manager
CLASSIFICATION: Full Time, Non-Exempt
PROGRAM/DEPARTMENT: Health Home Care Management
IMMEDIATE SUPERVISOR: Program Manager
POSITION SUMMARY: Works with people with qualifying Physical/Mental Health/Substance Abuse diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Provides ongoing advocacy through person centered approach
QUALIFICATIONS:
Minimum Education, Training & Experience:
Bachelor’s Degree in Human Service related field
Two (2) years experience providing Care Management services (Case Management) to individuals with qualifying factors; i.e. physical/MH/ Substance diagnosis
Functional knowledge of community resources
Preferred Education, Training & Experience:
The above plus two (2) years’ experience developing Care Plans
ESSENTIAL SKILLS AND ABILITIES:
Valid New York State driver’s license that meets the Agency’s driving policy
Ability to work independently in the community
Ability to work flexible hours and travel throughout Niagara County
Ability to work effectively and independently including address multiple tasks efficiently
Possesses strong organizational, analytical and creative thinking skills
Ability to effectively communicate verbally and in writing including developing rapport with the people we serve
Ability to represent self and agency in a professional manner
